This patient displays symptoms of angina, tachycardia, rash, and joint pains. This lupus-like syndrome is a well described adverse effect of hydralazine therapy.
The vasodilatory action of hydralazine can result in reflex tachycardia and decreased oxygen delivery to the myocardium in patients with existing coronary artery disease. Other agents known to cause a systemic lupus erythematosus-like syndrome include isoniazid, procainamide,and phenytoin.
